Nick felt trapped underwater, everything was different, wrong in this world he found himself in. It wasn't just the people, but places that should have been familiar were subtly off.
His house, where he'd lived for over a decade, was the worse. His coffee mug had been in the wrong cupboard and it had taken him hours to find his whisky. Somehow, it was these little things which had brought home just how different this world was.
Nick was struggling to surface, in the hope that maybe, just maybe, if he succeeded he'd wake up back in his own world.

It would be all the little things that would be horrible - the big things can be handled, but the little ones niggle so badly.
